Hospice of NW Ohio is Hiring a RN Homecare - Triage Near Toledo, OH

This position is 4, 10 hour shifts 1:00 p.m. - 11:00 p.m. and only 1-2 holidays a year!

NO WEEKEND OR ON CALL REQUIREMENTS!!

Qualification Requirements

  • Graduate of an accredited school of registered nursing; BSN preferred.
  • Currently licensed as an RN through the Ohio Board of Nursing; must be able to obtain a license in the state of Michigan.
  • Minimum 2 years of medical-surgical nursing experience preferred; previous experience in homecare, and/or hospice experience is preferred.
  • Has working experience charting in an EMR system; Netsmart system preferred.
  • Possesses clinical expertise related to critical thinking, assessment, problem-solving, and technical skills.
  • Possesses the ability to provide basic and skilled nursing care, set priorities of care, and appropriately assess limits of intervention.
  • Possesses strong verbal and written communication skills; demonstrates ability to communicate effectively with patients, families, and team members regarding care issues.
  • Has the ability to work independently and within an interdisciplinary team.
  • Demonstrates strong organizational, time management, and prioritization skills.
  • Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements.
  • Must be CPR certified according to the American Heart Association CPR standards.
  • Understands the hospice philosophy, principles of death/dying, concepts of pain/symptom management, and adult teaching/learning principles.

Essential Job Responsibilities

  • Responds to patient family needs and provides professional nursing care to patients and family by utilizing all elements of the nursing process including:
    • Contributes to the comprehensive assessment
    • Develops, collaborates, implements the plan of care in conjunction with IDT to meet patient/family physical, psychosocial, spiritual, emotional, and bereavement needs
    • Evaluates patient/family outcomes
    • Delegates care appropriately, provides supervision to LPN and HA and documents according to agency standards
    • Provides hands on patient care, medications, services, and prescribed treatments including those requiring professional nursing skills
    • Initiates appropriate referrals to other health disciplines and to other agencies for needed services
    • Teaches appropriate care, treatment, information and health practices to patients, families, caretaker(s), and others
    • Discharges patients according to agency policies
    • Obtains and processes physician orders
    • Maintains a safe environment for patients and uses equipment safely and properly
  • Maintains communication with appropriate team members/supervisors regarding patient care issues. Effectively communicates verbal and written patient/family information to IDT members.
  • Initiates appropriate referrals to other health disciplines and to other agencies for needed services.
  • Conducts admission/evaluation visits according to agency standards (shift dependent).
  • Provides support at the time of death, if in attendance, and assists with closure contacts as assigned.
  • Completes clinical documentation timely and accurately according to agency standards.
  • Effectively and efficiently manages resources (supplies, medications, and equipment).
  • Maintains infection control practices as related to patient care activities and agency standards.
  • Responsible for learning and utilizing technology made available by the agency to increase efficiency, communication and performance of duties.
  • Participates in the agency’s Quality Improvement program.
  • Attends staff meetings and completes all mandatory training.
  • Holds in strictest confidence all patient information and discloses information and data only to persons authorized by Hospice.
  • Performs other duties as assigned.
