El Paso is the only non-profit hospice organization in the city. We are the oldest (40 years) and largest hospice in the community. We were founded by El Pasoans for the sole purpose of providing specialized care, comfort and support to persons with a terminal illness, regardless of their ability to pay.
Our main focus is maximizing the remaining quality of life for the dying, while also providing support for their caregivers and loved ones. Specialized teams provide compassionate comfort and care and are comprised of physicians, registered nurses (RNs), certified nursing assistants, social workers, chaplains; plus admissions counselors and support staff.
The majority of these services are provided at home. Hospice care can also be provided in nursing, Hospice foster, and assisted living homes; along with rehabilitation centers and skilled nursing facilities.
The “Center for Compassionate Care” is the city's first stand-alone “hospice in-patient facility.” We provide acute hospice care in a quiet and comfortable setting. In this home-like setting, patients receive 24-hours of specialized skilled care when pain and symptoms cannot be managed in a home environment.

As a Hospice El Paso LVN , you will provide appropriate vocational nursing care as prescribed by the physician in compliance with applicable LVN licensure and agency policies and procedures to those patients who desire to receive hospice care.

Minimum Requirements:
· Current licensure as a Licensed Vocational Nurse in the State of Texas.
· Current CPR certification.
· Two years clinical experience. Background in care of terminal patients or community nursing preferred.
· Working knowledge of nursing procedures.
· Personal vehicle available for transportation during work.
· Valid driver’s license and insurance as required by the State of Texas.
· Must be able to read, write, and speak English at a level which will permit the employee to accurately understand and communicate information as needed to safely and efficiently perform job duties.
· Bilingual in the Spanish language preferred.
Requirements:Duties and Responsibilities:· Participates in the planning of the nursing care needs of an individual in cooperation with the other IDG members. Participates in the development and modification of the nursing care plan.
· Collects data and performs focused nursing assessments of the health status of assigned patients. Provides documentation within 24 hours of assessment.
· Assesses and documents current pain level and outcome with every visit. Complete pain assessment minimum of once per week on all patients assigned.
· Participates in health teaching and counseling for patient, family, and caregivers as appropriate to overcome identified knowledge deficits, documents in notes and home charts.
· Reports changes in patient’s condition to Clinical Manager or Case Manager and physician as appropriate.
· Obtains physician orders and implements interventions and changes in treatment as appropriate. Documents phone contacts on the same day.
· Monitors medication levels at each visit, ensuring the patient always has a sufficient quantity, and reports need for adjustments to physician to coordinate with patients declining status.
· Makes appropriate referrals to other disciplines, recognizing need for additional assistance.
· Conducts follow-up visits within 24 hours of Start of Care and assists in the evaluation of an individual’s response to nursing interventions and the identification of an individual’s needs.
· Initiates and carries out prescribed treatments and evaluates medication effectiveness as designated in the plan of treatment.
