Full Time position is eligible for a  ($5,000.00) Retention Bonus payable at $1,250.00 quarter for the first year.

HCP is looking to add a registered nurse to join our overnight team. This RN will primary work from home, answering phone calls from patients and families and will make visits as needed for overnight needs or concerns for our patients.

Qualifications required for this position include but are not limited to: 

• RN, with current PA license 
• Minimum of one year clinical nursing experience-hospice or home care experience helpful but not required
• Proficient at basic computer/keyboard skills
• BLS Certification (required)

Responsibilities of this position include but are not limited too:

• Triage calls that come in after hours
• Respond to patient needs either via phone or in person visits
• Pronounce patients at time of death
• Provide emotional support to patients
• Education and teaching to patients and families
• Ensure the continuum of 24/7 of nursing care
