Hospice El Paso is the only non-profit hospice organization in the city. We are the oldest (34 years) and largest hospice in the community. We were founded by El Pasoans for the sole purpose of providing specialized care, comfort and support to persons with a terminal illness, regardless of their ability to pay.
Our main focus is maximizing the remaining quality of life for the dying, while also providing support for their caregivers and loved ones. Specialized teams provide compassionate comfort and care and are comprised of physicians, registered nurses (RNs), certified nursing assistants, social workers, chaplains; plus administrative counselors and support staff.
The majority of these services are provided at home. Hospice care can also be administered in nursing, foster, and assisted living homes; along with rehabilitation centers and skilled nursing facilities.
The “Center for Compassionate Care” is the city's first stand-alone “hospice in-patient facility.” We provide acute hospice care in a quiet and comfortable setting. In this home-like setting, patients receive 24-hours of specialized skilled care when pain and symptoms cannot be managed in a home environment.

As a Social Worker, you will be a member of the patient care team providing social work services to all Hospice El Paso patients and their caregiver system. You will also be a member of the interdisciplinary team coordinating supportive services in a consistent, competent, and professional manner.

Minimum Requirements:
· Bachelor’s degree from an accredited school of social work.
· Minimum two years experience in a multi-disciplinary setting.
· Licensed by the State of Texas.
General Duties and Responsibilities:
1. Performs psycho-social assessment of patient and care giving system at start of care and establishes a plan of care.
2. Reports significant changes and newly identified problems in the patient’s and care giving system’s psychosocial status.
3. Provides emotional support, counseling services, and education to help meet the psychosocial needs of the patient and care giving system in relation to the terminal illness.
4. Facilitates caregiver access to necessary community resources with the goal of maintaining a stable care giving environment.
5. Provides initial and follow-up bereavement assessments to the Bereavement Coordinator in a timely manner. When necessary provides bereavement counseling to caregiver system following the death of a patient.
6. Provides appropriate documentation of all client visits and telephone contacts in a thorough, accurate manner within the time requirements outlined in policy.
