To be considered for this position you will need to provide proof of the following credentials and documentation:

  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • One year of experience in hospice/palliative care, public health, oncology, acute care, or home health nursing
  • Current and valid Texas RN License
  • Valid Texas state driver’s license (auto liability insurance required) with a qualifying driving record.

Position Summary

The successful candidate will be responsible for providing patient/family care in consultation with Primary Physician, Team Manager, and Interdisciplinary Team (IDT) in accordance with Hospice Care Partners' philosophy and delivery of care. Provides appropriate nursing interventions and timely, accurate documentation. Assists patients/families with external resources such as home health, financial concerns, counseling, etc. Educates and motivates patients and families regarding actions and behaviors related to end-of-life care and identifies special needs. Must have reliable transportation and automobile insurance.

This position requires excellent customer service skills, the flexibility of availability for Mon thru Fri coverage, typically 08:30 am to 5:00 pm, and on-call need for after hours and weekends on a rotating schedule. Ideally, we are looking for individuals with one or more years of experience in a hospice field nursing position who possess leadership experience.


Required Skills

  • Knowledge of health care community, regulations, and resources
  • Knowledge of nursing assessment for home care
  • Knowledge and understanding/acceptance of Palliative Care and Hospice Philosophy
  • Knowledge of agency medical record documentation system and process
  • Able to provide skilled nursing intervention for patient/family with leadership to the team as required by state/federal regulations and certification
  • Able to communicate effectively and compassionately with team members, patients/families, and community
  • Able to educate patients/families and community about hospice, palliative care, and Hospice Care Partners’ philosophy and nursing care
  • Able to demonstrate flexibility, dependability, initiative, professionalism, and maturity
  • Able to manage time appropriately to meet required visit frequencies with a caseload
  • Able to meet timely documentation requirements of the position
  • Able to function effectively with assigned technology
  • Able to operate independently and with integrity in the role

Required Experience & Minimum Qualifications

  • Current and valid Texas RN License
  • Valid Texas state driver’s license (auto liability insurance required) with a qualifying driving record.

  • One year of experience in hospice/palliative care, public health, oncology, acute care, or home health nursing

Preferred

  • Bi-lingual Spanish/English
  • Baccalaureate degree in Nursing
  • Two or more years of experience as a nurse
  • Case Management nursing experience
  • Home Health nursing experience
  • Experience with Consolo EMR program a plus.

Physical Requirements:

  • Prolonged periods of standing and walking.
  • Must be able to lift up to 75 pounds at a time.
