As a Patient Engagement Specialist at Horizon, you will…

• Handle referral intake calls for all of Horizon Village Campus and Terrace House
• Provide excellent customer service and phone support; triage company calls
• Review incoming referrals from external and internal sources
• Manage the residential waitlists by clearly documenting correspondence
• Coordinate admissions based on bed availability while ensuring continued communication and follow-up in order to manage arrival and timing of intakes
• Verify eligibility and benefits through EPACES, HEALTHeNET and PVerify

• Work closely with medical organizations, private facilities, physicians, law enforcement agencies, clinicians, program directors and insurance companies to coordinate treatment for patients

• High School Diploma/equivalent required 
  • Bachelors preferred
• 2 years of experience in customer service or in a behavioral health setting required

Location:

6301 Inducon Dr E, Sanborn NY

Hours:

Monday – Friday: 8:00am - 4:30pm. Rotating weekend schedule required.

Compensation 

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $19.75 - $21.45. Compensation adjustments are evaluated annually. 

Physical demands associated with this position include:

• Sitting for extended periods, alternating between standing and sitting
• Mobility required with repetitive wrist, hand, and finger movements
• Handling light duties, occasionally lifting objects up to 20 pounds
