Hopewell Health Centers is a nationally accredited federally qualified health and community mental health center with 23 sites across TEN counties throughout Southeastern Ohio (Athens, Franklin, Gallia, Hocking, Jackson, Meigs, Perry, Ross, Vinton, and Washington). We are currently seeking an Adult CPST Team Lead to join our Athens, OH Behavioral Health team.

As a CPST Team Lead, you will deliver quality, person and family-centered, medically necessary, rehabilitative, culturally-competent Community Psychiatric Supportive Treatment interventions including but not limited to: assessment of needs, skills building/empowerment, crisis management and intervention, advocacy, education/training, eliminating barriers, coordinating/linkages, and monitoring. Some specific responsibilities include but are not limited to:

• Ensure that all cases are seen and followed in a timely manner in accordance with clinic access guidelines/clinical need
• Assign/reassign cases
• Facilitates team meetings on a regular basis
• Reviews cases on a weekly basis with staff and as needed with Administrative Supervisor
• Liaisons with community partners: GP, AMHA, 317 Bd, Athens Homeless Coalition, ISS, HRS, Medical community, SSA, JFS, food banks, etc.
• Complete yearly clinical evaluations
• Directs/Provides emergency back up to ACPST providers

What do we look for in our CPST Providers??

• Bachelor’s Degree, LSW, RN, or PC (Bachelor’s in Social Work or closely related field is preferred)
• Demonstrated the skills and ability to communicate and work with persons receiving service, their families, community service providers, and team members in the assessment, problem solving, and delivery of rehabilitative services
• Good verbal and written communication skills
• A valid driver’s license, good driving record, and vehicle suitable for transportation. (Must have a minimum of 3 years driving experience and be at least 21 years of age)
• Must be personable, reliable, and organized.
