JOB DESCRIPTION: REGISTERED NURSE

Status: Non-Exempt

Reports To: Chief Opertating Officier

HOPE Services Hawaii is seeking a full time nurse to work as part of the Street Medicine Team and clinic with direct oversight of Chief Operating Officer. Working as an integrated and multidisciplinary team that seeks out the men and women of our homeless community, offering both urgent and continuous medical and behavioral health care. To succeed in this role, you must be a flexible, compassionate, and self-motivated individual who embraces the mission. You are committed to delivering low-threshold, high-quality nursing and care management in various non-traditional locations, to a vulnerable population of individuals with a history of chronic street homelessness.

Primary "office" may be in an outdoors community setting, homeless shelter, or resource center.

Using a multi-disciplinary skill set, the Registered Nurse must be able to exercise the following skills:

  • Accompany Street Medicine Team on weekly Wednesday night missions, and Outreach Team on weekly morning missions (1x per week). Assesses, treats and educates patients in need of medical assistance, provides follow up evaluation and care as needed. i.e. face to face on street, telemedicine, follow up appointment in clinic, prescription medication delivery.
  • Provides health and wellness assessments on a rotating basis at HOPE Shelters once per week. (this may sometimes be accomplished via ZOOM). Assesses, treats and educates patients in need of medical assistance, provides follow up evaluation and care as needed. i.e. face to face on street, telemedicine, follow up appointment in clinic, prescription medication delivery.
  • In coordination with Street Medicine program provide medical assistance to HOPE outreach team and shelter sites as necessary.
  • Documents all services provided and participates in tracking and reporting activities to support effective management of the program.
  • Provides in-service trainings to increase staff knowledge of health care issues impacting the target population and to promote effective communication and care.
  • Engage patients with substance uses disorder and mental illness, with a strong commitment to harm reduction principles and trauma-informed care.
  • Track appointments, referrals, and primary care activities and follow up with the patients to assure that they keep their appointments or reschedule if the appointment is missed.
  • Attends mandatory meetings for the purpose of: Continuing Education, Program Development, Case Management, etc. as designated by Chief Operation Officer.
  • Performs other duties as deemed necessary by supervisor.

Other duties:

  • Physical, functional, psychosocial and cognitive assessment of patient
  • Provide support to Medical providers, assist with administering medications to patients or out in the community
  • Review of hospital shelter referrals before clearing into shelters to ensure we address appropriateness of level of care
  • Establish achievable goals and intervention to be integrated into the individual’s Master Recovery plan
  • Promoting and empowering meaningful choices and service access options, as well as allowing the service participant to influence the type, duration, frequency and intensity of supports;
  • May have a caseload of patients who need medication management
  • Facilitate collaboration between the patient and the multidisciplinary team to increase patient capacity for independently managing medical and behavioral health conditions.
  • Evaluate patient’s response to the care based on continuous assessment and analysis of nursing interventions and care
  • Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments to patients within home and community situations.
  • Identify patient’s need for health education or coaching and provide as appropriate or link with community resources
  • Identify any underlying substance abuse disorders and mental illness and encourage access to treatment resources including counseling, therapy and recovery programs.
  • Nursing documentation that meets agency standards
  • The right to refuse or revoke services and/or seek restitution or grieve instances where they feel services are inappropriate or inadequate.
  • Maintain HIPP compliance with management of protected health information
  • Assist in arranging for ongoing prescribed long acting antipsychotic injections for patients and assist with administering when needed
  • Support laboratory monitoring of medication efficacy by establishing prompts according to community standards.
  • Nursing support for ambulatory detox when assigned
  • Provide clinics or classes to support individual self-management of chronic health conditions, prevention of communicable diseases and general health maintenance.
  • Participate in development of treatment protocols with medical-clinical team

The Registered Nurse must have a considerable knowledge and expertise in the areas of:

  • Legal requirements and risks to perform this type of work, including the relevant legislation that impacts decision-making in particular situations;
  • Rental housing and requirements of tenants and landlords by law;
  • Homelessness, especially chronic homelessness;
  • Dependency and addiction to substances;
  • Aboriginal culture and impacts on cultural identity;
  • Economic poverty;
  • Income support services;
  • Child welfare;
  • Health care, mental health care, and addiction services;
  • Harm reduction;
  • Trauma and abuse;
  • Domestic and intimate partner violence (victim and perpetrators);
  • Life changes and ageing;
  • Fetal alcohol spectrum disorders;
  • Brain injuries;
  • Privacy and confidentiality;
  • Self-care;
  • Corrections and criminal justice.

In the course of performing the duties of the Registered Nurse it is not uncommon to see, engage or be confronted with first hand – or have staff encounter first hand – the following: violence and threats of violence; profane, racist and/or sexist language; bodily fluids; conflict; interactions with First Responders; alcohol and other street drugs; cigarette smoke; death of service participants or her/his associates; nudity of service participants or her/his associates; friends/family dynamics with service participants; people involved with sex work; people involved in the drug trade; persons used against their consent, will or knowledge; people in conflict with the law; and/or other situations that may be unsettling. Measures are taken to train staff to appropriately deal with these situations, but those in the position should reasonably expect these types of things to occur and the Team Leader must provide appropriate direction and support to these situations.

Minimum Qualifications:

  • State of Hawaii RN license
  • Two years of nursing experience
  • Understand substance use disorder and behavioral health issues
  • Interest and experience in caring for underserved populations.
  • Past experience working with vulnerable populations, and a desire to bring one’s whole self to the service of our community
  • An ability to remain peaceful and productive in times of stress, and to collaborate effectively in a multicultural community

Candidates must display and/or demonstrate physical ability to:

  • Engage with a computer keyboard and look at a computer monitor for several hours per day, and/or, exercise with comparable technologies like tablets;
  • Climb and/or walk up and down stairs and/or walkways.
  • Walk and/or standing for up to 30 minutes at a time.
  • Sit for periods of up to 30 minutes at a time.
  • Lift, carry, pull and/or push items up to 30 pounds at a time and for short distances. Such motions may include reasonable bending, stooping, reaching and twisting.

Starting Pay: $34.38/hour
