We are currently searching for a Full Time Social Worker to join our Garland and Greenville teams.
Responsibilities:
- Serve as a patient, family, and team advocate.
- Clearly and effectively communicate the value of hospice & palliative care service to other team members, patients and families.
- Have the desire to work with those at the end of life.
- Provide professional psychosocial and emotional support.
- Serve as a resource for the medical team.
- Participate in the patient care planning process and promote coordination of patient care.
- Service Greenville Service Area.
- Education: Must have an active Social Work license from an accredited school of Social Work.
- LMSW/LCSW preferred.
- At least two years experience in a clinical care setting that may include: hospice, hospital, long-term care, or clinic settings.
- Great interpersonal skills, both verbal and written.
- Familiarity with an EMR (Electronic Medical Record).
- Service Greenville Service Area.
- Monday to Friday.
- Master's (Required).
- Medical Social Work: 2 years (Required).
- Social Work License (Required).
