JOB TITLE: CNA/DSP SHIFT COORDINATOR

SUMMARY: Oversee the community’s direct care staffing needs by overseeing performance and assisting the direct care staff to ensure efficiency and smooth workflow.

ESSENTIAL DUTIES AND RESPONSIBILITIES: An individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or abilities required. Other duties as assigned.

  1. Provides daily direct care and support to residents at the level identified in his/her individualized care and/or education plan. This includes bathing, dressing, grooming, food consumption, transferring and ambulating. Provision of care also includes providing comfort to residents by creating a positive home-like environment; helping the residents develop trust; offering emotional support; assisting with daily routines, working with residents’ families; and communicating and actively listening. Monitors and reports to nurses or QIDP (Qualified Intellectual Disabilities Professional) change in resident condition or other relevant matter(s).
  2. Provides for the residents’ health needs, including but not limited to: determining measurements accurately as assigned (vital signs, intake, output and food consumption, etc.); ensuring proper documentation is maintained; assisting the nurses during treatments, medical procedures and emergencies; recording accurate, legible information about resident care and condition per facility practices; and responding to any call light or emergency alarm timely.
  3. Provides supports and active treatment to residents by assisting in the development and implementation of all Support Plans including assisting residents in achieving their personal goals and desires, providing interaction and choices for activities that support a meaningful day, and support residents to be independent in activities of daily living, socialization and leisure skills.
  4. Maintains employee and resident safety. Duties including but are not limited to following infection control practices; maintaining a clean and safe environment; report safety concerns immediately, performing procedures correctly; timely report injuries to Administrator; participating in fire and disaster drills. In an emergency, carries out assigned duties to assure client and employee safety.
  5. Develops and organizes employee daily assignments, resolving staff concerns and requests, coordinating with the DON (Director of Nursing), the ADON (Assistant Director of Nursing), Nurses, QIDP’s and/or human resource department, establishing and enforcing guidelines, and maintaining an active communication line with team, all while implementing the company’s policies and regulations.
  6. Identifies staff development and training needs and ensures that training is obtained.
  7. Oversees and/or assists with clinic and resident appointments as needed. Assign drivers for appointments and be available to drive as needed.
  8. Assists with the planning, implementation, and evaluation of plans for care for assigned residents and reports information and suggested approaches to QIDP and nurse.
  9. Carries out administrative functions, including but not limited to: prioritizing daily tasks, participating in staff meetings, regularly reviewing, and implementing updated policies and procedures, attending continuing education programs, and required in-service education, and applying information learned to current job responsibilities.
  10. Assign weights and lengths of residents as ordered.
  11. Find coverage for staff call offs.
  12. Maintain and update schedules as needed.
  13. Maintain and update two person transfer list.
  14. Stock CNA/Nursing supplies when needed in nurse storage area.
  15. As directed, performs quality assessment and assurance functions.
  16. As directed, assists with survey processes.
  17. This position requires the ability to respond to common inquiries or complaints from resident’s families, regulatory agencies, or members of the community and the ability to speak, understand and read the English language to the extent necessary to care for residents safely and properly.
  18. This position requires excellent interpersonal skills with an elevated level of energy and enthusiasm; good verbal and written communication skills; and the ability to organize, document, and implement detailed programs.
  19. Maintain professionalism in dress, language, and conduct when representing the residents and facility.
  20. Assume accountability for data contained in the Employee’s Handbook.
  21. This position requires standing, sitting, stooping, pushing, pulling, lifting to 50 pounds or more, bending, climbing, twisting upper body, walking, running, carrying, listening, talking, use of telephone, and exposure to medical equipment and persons with risk of illness.

SUPERVISORY RESPONSIBILITIES: Indirectly supervises direct care staff as their job functions relate to resident care and carries out supervisory responsibilities in accordance with the facility policies, practices, procedures, and applicable laws. Responsibilities may include: training employees; planning, assigning, and directing work; appraising performance; addressing complaints and resolving problems. Must consistently use discretion and independent judgement.

EDUCATION and/or EXPERIENCE: High school diploma or general education degree (GED) per department requirements. Meets state and federal requirements. Computer literacy required.

CERTIFICATES, LICENSES, REGISTRATIONS: Successful completion of a department approved Nursing Assistant (CNA), Habilitation Aide (CHA), Direct Support Person (DSP) program, or training and competency evaluation program, including passing of the certification exam per department requirement. Persons enrolled in a department approved program must successfully complete the program no later than 120 days after the date of initial employment. CPR, First Aid, Driver’s license per department requirements.

SUPERVISORY RESPONSIBILITIES: As directed, supervises employees and carries out supervisory responsibilities in accordance with the facility policies, practices, procedures, and applicable laws. Responsibilities include, training employees, planning, assigning, and directing work; appraising performance, addressing complaints and resolving programs. Consistently uses discretion and independent judgement.
