Honor Community Health is Hiring a Behavioral Health Clinician (SUD) Near Pontiac, MI

Honor Community Health is a 501c3 Federally Qualified Health Center co-located within 20 locations. Our mission is to provide for the health and wellness needs of the underserved of Oakland County through the provision of comprehensive, integrated primary, behavioral health, and dental care. Position Description

Reporting to the Director of Strategic Initiatives, the full time, exempt Behavioral Health Clinician, Certified Community Behavioral Health Clinic (CCBHC Program) is an essential member of a high performing healthcare team. The Behavioral Health Clinician (BHC), CCBHC Program will provide evidence-based clinical behavioral health services in accordance with the requirements of the CCBHC Program Requirements. The BHC, CCBHC will uphold Honor Community Health’s mission and quality standards which result in better patient health outcomes, overall satisfaction, and stronger operational and fiscal capacity of the health center. The BHC, CCBHC will adhere to Honor Community Health (HCH) policies and procedures while carrying out and monitoring a wide range of clinical processes. The BHC CCBHC will maintain strong internal and external relationships, serving as liaison on behalf of HCH’s centers. The BHC, CCBHC works collaboratively to create and maintain a culture of excellence and dedication to providing compassionate and high-quality behavioral health care to all. Productivity expectation of a trained BHC, CCBHC is an average of 10 or more behavioral health consultations/counseling per clinical day..

Essential Functions

  1. Assessment
  1. Conducts assessment of the presenting issue that is descriptive considering frequency, intensity, and environmental context in which the issues occur.
  2. Diagnosis formulation is clear and includes significant etiological factors, identify key difficulties, includes history of symptoms, onset, and precipitating factors.
  3. Identify the level of care that is appropriate for the individual’s presenting issues.
  4. Considers necessary interventions based on evidence-based practices.
  5. When necessary, clinician will evaluate for risk of harm, current and past functional levels, support system within the individual’s environment, and explore co-morbid/co-occurring conditions.
  6. Limited use of screening and assessment as appropriate. Individual, couples, and/ or family counseling with patients, including.
    1. Functional and strength-based assessment and diagnosis,
    2. Psychoeducation for patients and their support systems,
    3. Medication adherence counseling and disease self-management counseling,
    4. Motivational Interviewing to develop behavioral strategies aimed at symptom reduction,
    5. Brief problem solving and solutions-focused interventions aimed at modifying negative thinking and promoting self-efficacy.
2. Treatment
  1. The goals are in the individual/guardian/family’s words and intentions based on individual’s understanding of the assessment, findings, and recommendations of all his/her/their support systems. The treatment is based on evidence-based practices.
  2. Goals and objectives are written as attainable, measurable, have observable outcomes, realistic, written in positive terms, build upon abilities/strengths, preferences, needs, and they embody hope/alternatives to a current situation.
  3. Develops treatment plans and reviews as needed/indicated.
  4. Crisis Intervention – Be immediately available to the health center staff during working hours in the event of a psychiatric emergency. Participate as a member of the health center staff in the event of any other kind of emergency.
 
  1. Coordination of Care
  1. Accepts referrals from the medical provider regarding patient’s behavioral needs.
  2. Provide consultation to and coordinate care of patients with health center primary care staff. Identify, refer, and advocate for patients needing specialty behavioral health service, and other services as needed. Assist in coordination of services when needed.
  3. When needed the BHC will consultation with other community partners such as school counselor, and other mental health professionals to enhance understanding of the patient, provide decision support for treatment planning and assist in the implementation and monitoring of biopsychosocial treatment plans.
  1. Documentation
  1. Charting – Complete all charting as needed and required by contacted funding sources: including assessments, progress notes, and billing. Charting should be completed in a timely fashion, no later than 72 hours.
  2. With appropriate medical release, works with other community partners to develop a plan of care for each patient.
  3. Documentation of progress reflects intervention, skills used to cope/adapt/respond/problem solve. Reinforce new behaviors and strengths. Identify specific skills that are taught/modeled/practiced. The Intervention elements of the progress note shall describe the following: clinician’s interventions, clinician’s assessment, including a risk assessment when applicable, document advice/recommendations given to individual and family, response to the interventions and progress or lack of it towards goals and next steps.
 
  1. Quality Improvement
  2. Participate in local/national behavior health trainings and conferences.
  3. Assist in provider training and education in the primary care behavioral health model.
  4. Facilitates and coordinates care of patients to ensure quality outcomes. Participate in quality improvement activities and actively participate in health center meetings related to integrated care.
  5. Performs other duties as assigned. During a public health emergency, the employee may be required to perform duties similar to but not limited to those in the job description.

What are we looking for?

  • Current license to practice in Michigan as a Licensed Clinical Social Worker, Limited License Psychologist, License Professional Counselor, Doctoral degree in Psychology. 
  • Limited Licenses may be considered (LLMSW, LLPC, LLC, TLLP) 
  • A Master’s degree in social work or related field 
  • Minimum of two years’ clinical experience with at least one year of clinical experience treating patients with substance use disorders.
  • CAAB/ substance use experience preferred
  • Knowledge of primary care and healthcare integration is strongly recommended
  • Must maintain BLS Certification through the American Heart Association.
  • Experience with Federally Qualified health Centers is helpful.
