Home Well Care Services of Durham- Chapel Hill, NC is looking for Registered nurses to work part-time or PRN based on assignments. The RN will be responsible for visiting the home and processing the care plan for each client.

MINIMUM QUALIFICATIONS:

• Registered Nurse License

Additional qualifications:

• A reliable means of transportation with current insurance and a valid driver’s license.
• Must possess excellent written and verbal communication skills and be able to interact effectively with a diverse audience including clients, families, care providers, and other professionals.
• Passionate about helping others live with dignity.
• Must present a positive and professional business image.
• You will need to pass TB test or Chest Xray.
• Must pass Bloodborne Pathogen course
• Must have Hepatitis B vaccination or willing to do so.
• Must pass a criminal background check.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Conduct initial assessment.
• Develop plan of care.
• Regularly communicate with the client and/or designated client representative to follow up on service delivery and ensure client satisfaction.
• Perform scheduled and non-scheduled in-home client support visits.
• Conduct reassessments according to the agency’s established frequency and as needed due to a change in client condition or recent event.

PHYSICAL AND ENVIRONMENTAL DEMANDS: Travel to the client’s place of residence using a reliable means of transportation.

POSITION TYPE & EXPECTED HOURS OF WORK: Part-Time to start, Full-Time as the agency grows.
