POSITION OVERVIEW:
Employees are employed by HIL through an employer of record agreement, providing services in conjunction with Waukesha County Human Services. This position is responsible for addressing areas of housing and benefit needs that are necessary to maintain a stable living environment for persons with serious and persistent mental illness.
Our roles are competitively compensated with a starting wage of $40,000 - 46,000 annually. Paid out bi-weekly.

ESSENTIAL FUNCTIONS:
  • Receive referrals for persons with serious mental illness in need of benefit assistance, housing, and/or assistance in learning self-care or independent living skills.
  • Work to build a trusting relationship with clients, acting as a resource and problem-solving helper, promotes independence and recovery principles.
  • Maintains individual dignity and exercises courtesy when assisting/working with people.
  • Provides assistance with money management including opening and managing bank accounts, acting as a payee, monthly check requests, and distribution, working towards increased independence.
  • Provides assistance in application process for enrolling clients in benefit programs such as Food Share, Medicare D, Medicaid, homestead and tax filings. Completes initial application, reviews and appeals.
  • Verifies Pharmacy Benefits and provides guidance for keeping benefits; where indicated, completion of PAP, and QMB.
  • Acts as a liaison with Social Security, Fiscal and outside financial interests.
  • Assistance with applications for rent subsidies, finding and keeping suitable housing, developing ongoing relationships with landlords, coordination of matters pertaining to moves to new living arrangement: organizing physical move and acquiring necessary household items.
  • Address housing and personal care needs and in conjunction with case manager/service facilitator, in selective instances, time management and socialization needs through 1:1 meetings in the community to teach, assist and monitor skill level in keeping with treatment plan goals.
  • Work to promote health and wellness: proper nutrition through grocery shopping and meal planning as needed per treatment plan goals.
  • Assists in making transportation arrangements for medical and dental appointments as needed and coordination between medical providers and the CCS/CSP team to assist with follow through with treatment recommendations.
  • May provide case management services as needed. This includes assessing functioning and need for services, working in collaboration with the client to develop treatment/recovery plan and adhering to Administrative Code DHS 63 standards, including timely documentation of progress notes per treatment plan, reviewing and updating the recovery plan every 6 months (or earlier if indicated), completing annual functional screens, formulating and submitting recommitment letters, and overall maintenance of clients charts are to align within the established policies and procedures.
  • Other tasks as assigned
  • Meets periodically with HIL management as requested.
What you will bring: Case Manager:
  • Empathy and compassion for the clients we serve. 
  • Bachelor's Degree from a recognized college or university with a major in sociology, social work, psychology or closely related field, Master's preferred.
  • Bachelor's Degree from a recognized college or university with at least four courses in sociology, social work or psychology, plus one year of professional experience in the field of mental health, Master's preferred.
  • 1000 hours of experience in the field of mental health, with services delivered specifically to the chronically mentally ill and their families - OR - Two years of comparable experience in a mental health setting.
