As a Nurse Practitioner for Homeland Hospice, you would contribute to our community of caring by providing direct support to patients and family members during a significant part of their life journey.

Homeland Hospice provides compassionate end-of-life care to help patients and families make the most of their moments together.   • This is a salaried full-time, Monday through Friday, Daytime position.
  • This position requires valid Pennsylvania CRNP and Registered Nurse licenses, a valid Pennsylvania driver’s license, and reliable internet access.

As a full-time Homeland Hospice Nurse Practitioner, you would be eligible for:

Essential Duties and Responsibilities include the following.

Patient Evaluation

  • Reviews patient's medical eligibility for palliative care services by performing a nursing examination and documenting the same in the clinical record in order to gather clinical findings to verify the patient’s continued medical eligibility for palliative care.
  • Performs and documents each face-to-face encounter and each subsequent recertification encounter no more than thirty (30) calendar days prior to the start of each benefit period.

Intervention/Patient Care Coordination

  • As needed, performs medical examinations and patient home visits beyond those required for the face-to-face encounters.
  • As directed, may assist in managing the patient’s terminal condition by coordinating with the attending physician or Medical Director.
  • Provides consultation for palliative and end-of-life care issues on an as-needed basis and as directed by the Director or the Hospice Medical Director.

Leadership/Professionalism

  • Maintains current knowledge of palliative care/hospice principles and practices.
  • Develops and maintains cooperative working relationships with palliative care/hospice staff.
  • Able to exercise independent judgment, critically analyze situations and make recommendations using appropriate problem-solving skills.
  • Able to function effectively as a member of a team.
  • Must have excellent interpersonal skills and the ability to work and communicate within all levels of the organization.
  • Effectively communicates in interpersonal, professional and public relationships.
  • Participates in the Quality Assessment/Performance Improvement Program of the agency as appropriate and needed.

Requirements:

Education

  • Nurse Practitioners must have a master’s degree or higher to qualify for certification in Pennsylvania.
  • Education includes advanced studies in anatomy, physiology, chemistry, psychology, nursing and microbiology. Master’s programs include a combination of classroom and clinical studies. Master’s nursing students study advanced health assessments to prepare for the diagnosis and evaluation duties of a practice nurse.
  • National certification requires the candidate to successfully pass an examination.

Experience

  • Minimum of two (2) years’ experience as a Nurse Practitioner.
  • Must have understanding of the palliative/hospice program of care.

Licensure, Registration, and/or Certification

  • Currently licensed and nationally certified as a nurse practitioner per the Commonwealth of Pennsylvania’s Board of Nursing (CRNP)
  • The candidate must have submitted proof of 45 hours of advanced pharmacology education. The course must have been part of an advanced program and not pharmacology courses that are part of the registered nursing program.
  • Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania requirements.
  • Satisfactory criminal background checks.
  • Must have reliable internet access.
