Responsibilities:

What You Will Do:

  • Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
  • Manage team of individuals who provide patient care
  • Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
  • Manage emergency situations in a prompt, precise, and professional manner.
  • Participate in the agency’s performance improvement program.
  • Provide nursing care in accordance with applicable regulations and Company policy.
  • Prepare and maintain accurate and complete clinical and progress notes on a timely basis.
  • Other duties as needed to ensure the health and safety of patients.
Qualifications:
  • Active RN state license
  • Valid drivers’ license
  • Five or more years of applicable nursing experience
  • Minimum of 1 year supervisory/leadership experience
  • Home Health Experience preferred.
  • Knowledge of home health, private duty nursing, Medicare, Medicaid, Ohio Home Care Waiver required.
  • Eligibility for certain benefits may depend on employment status
