Home of New Vision is a non-profit substance use, recovery services and co-occurring disorders treatment program located in Ann Arbor, Jackson and Ypsilanti, Michigan.

Home of New Vision has a full-time position available as described below. This position functions as a case manager in the substance use disorder Women's Specialty substance use treatment program (s) in Ann Arbor. This individual will assist clients in meeting the needs of their children, such as referrals to a pediatrician, access and referrals to daycare, referrals for housing needs and clothing, referrals to psychologist, and much more.

EXPECTATIONS: This position requires the employee to have the ability and common knowledge to work with both internal and external community resources to develop relationships to better serve the client. The employee is required to have the ability to work both independently and collaboratively to achieve the goals of our clients and organization. The employee is required to have and maintain working knowledge of laws pertaining to confidentiality, domestic violence, child neglect/abuse, welfare reform and licensing.

Responsibilities include:

• Facilitate intake for new clients.
• Directly assists clients in securing and maintaining needed services.
• Completes case management plan reviews, with clients, every 3 months.
• Documentation in medical records, according to the policies and procedures of Home of New Vision. Use of EMR.
• Aftercare planning and follow-up services.
• Assists with client transportation and childcare during Home of New Vision appointments, as needed.
• Participates in general staff activities (i.e. staff meetings and clinical case consultation).
• Other duties as assigned by Clinical Director or Program Manager

POSITION QUALIFICATIONS: A Bachelor Degree in social work or a related field preferred or education and experience. Two years' experience performing case management or similar service, preferred, along with understanding of substance use disorders, co-occurring disorders, and working with homelessness.
