What You'll Do:

• Conduct supervisory Client assessments.
• Develop and implement individualized care plans.
• Collaborate with other Health Care Providers
• Ensure compliance with healthcare regulations.
• Facilitate effective communication among Management Team, patients, families, and healthcare providers.

Qualifications:

• Active LPN license in Georgia
• Minimum of 3 years of home care experience, with at least 1 year in a supervisory role
• Strong clinical skills and knowledge of home care practices
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate and empathetic attitude toward clients and their families.
• Reliable transportation and valid driver's license.
• Clear background check and drug screening.
