What You'll Do:
Conduct supervisory Client assessments.
Develop and implement individualized care plans.
Collaborate with other Health Care Providers
Ensure compliance with healthcare regulations.
Facilitate effective communication among Management Team, patients, families, and healthcare providers.
Foster a positive and supportive environment for clients, families, and team members.

Qualifications:
Active LPN license in Georgia
Minimum of 3 years of home care experience, with at least 1 year in a supervisory role
Strong clinical skills and knowledge of home care practices
Excellent communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Compassionate and empathetic attitude toward clients and their families.
Reliable transportation and valid driver's license.
