Description
Under the direction of the Director of Clinical Operations, the Patient Care Manager supervises nursing staff to ensure the coordination of quality home health services and directs the multidisciplinary staff in effective case management. Responsible for working within the agency budget and in working towards the best possible reimbursement under Value Based Purchasing. Develops collegial relationships with internal HCS departments, referral sources, and the community. Assists in the development and implementation of staff education and orientation.
Requirements
- Graduate of a National League for Nursing approved program with a Bachelor of Science Degree in Nursing; or Graduate of an approved program in physical or occupational therapy;
- Three (3) years of satisfactory experience as a community health nurse, physical therapist or occupational therapist in a licensed and/or Medicare Certified home health program;
- Two (2) years supervisory experience; or
- A combination of education and experience that is determined to be equivalent to the above listed education and experience requirements;
- Possession of a current State of New Hampshire license to practice as a Registered Professional Nurse, Physical Therapist or Occupational Therapist;
- Maintain current cardiopulmonary resuscitation (CPR) certification;
- Excellent organizational skills;
- In-depth knowledge of OASIS Data Set as verified by a current Oasis Certification or Oasis certification is obtained within 12 months of hire;
- Valid drivers’ license with access to an automobile during working hours that meets the State of New Hampshire registration regulations and auto liability insurance;
- Must be able and willing to learn to utilize a portable electronic field device. Must be able to manage electronic patient health information (ePHI) remote access in accordance with HCS policies and procedures.
