Overview:
We are seeking a highly organized and detail-oriented individual to join our team as an Intake Coordinator. As an Intake Coordinator, you will play a crucial role in ensuring the smooth and efficient intake process for our patients. This position requires strong communication skills, the ability to multitask, and a solid understanding of medical terminology and Home Health requirements.

Duties:

- Communicate with care facilities to retrieve the correct information pertaining to receiving home health care.
- Conduct intake assessments with clients to gather necessary information
- Assist in coordinating and scheduling new patients to begin services with the correct discipline
- Collaborate with healthcare professionals to ensure accurate and timely intake procedures
- Maintain client records and ensure confidentiality
- Provide support to patients and their families throughout the intake process
- Assist in discharge planning and coordination of care
- Stay up-to-date on Home Health patient requirements for service

Qualifications:

- Must have and maintain a current LPN License in the state of Kansas
- Previous experience in a healthcare or medical intake
- Strong organizational skills with the ability to multitask effectively
- Excellent communication and interpersonal skills
- Attention to detail and accuracy in documentation
- Ability to maintain confidentiality of sensitive information
- Proficient computer skills, including electronic medical record systems

Pay: $22.00 - $25.00 per hour
