Duties -
A. Staff to support the individual to perform social and community integration tasks/activities to include: (1) walking in the neighborhood (2) shopping in favorite store and mall (3) going out for senior citizen functions (4) enjoying the local parks (5) attending community gatherings (6) engaging in other social and community integration activities/event of interest.
B. Staff to support the individual to perform activities of daily living (ADL) tasks/activities to include: (1) home cleaning tasks, shower, laundry, house chores (2) meal preparation or use of restaurant for lunch/dinner. (3) Other ADL activities of interest.
C. Staff to support individual perform health and wellbeing tasks/activities to include: (1) attend medical appointments as needed (2) Use of house in house games, leisure and entertainment (eg. Puzzles, watching TV), (3) Outdoors activities (4) hand and body hygiene (5) Get a haircut and other health and wellbeing activities of interest.
Qualifications - High school diploma or equivalent required; additional education or certification in a related field is a plus - Experience working with individuals with disabilities, autism, dementia, or other special needs is preferred. Valid driver's license and reliable transportation for transporting individuals to appointments or activities - Ability to follow instructions, maintain confidentiality, and adhere to safety protocols.
Familiarity with electronic documentation systems is a plus
