SUMMARY OF JOB:

Assists in meeting the psychosocial needs of patients/families and enables patients to utilize health care in order to achieve their optimal level of health.

RECRUITMENT REQUIREMENTS:

·Master's degree from an accredited school of social work.

·1-2 years Post graduate degree, experience in a health care setting, (hospital/home care preferred).

·Flexible, sound judgment, customer oriented, organized, keeps professional demeanor at all times.

·Excellent verbal and written communication skills.

·Available to work flexible hours, with weekends and holidays included.

·Knowledge of general disease process.

·Knowledge of family dynamics and interpersonal relationships.

·Able to travel to patient's home within service area.

·Able to use office equipment; i.e. beeper, car phone.

LICENSE AND REGULATORY REQUIREMENTS:

·Current license in state of practice required

·Valid driver's license.
