Position Overview:

Intermediate level position, which oversees shelter and/or motel Participants and activities using Evidenced Based and Trauma Informed case management methods to facilitate and motivate Participants toward permanent housing and positive change. Maintains a caseload of single individuals and/or families and supporting them in obtaining permanent housing. Also serves as primary liaison for Hotel/Motel program.


Responsibilities:

  • Practices the Agency’s treatment model, ethics, standards of conduct and procedures.
  • Committed to continuing professional growth by reading various forms of Agency or professional literature, attending scheduled Agency orientation, training classes, workshops, and cross training by observing groups and staff.
  • Reports all program information to supervisors or assigned persons based on policies and procedures, including Coordinated Entry staff or other program teams to be used for housing and program planning. This will include emotional challenging situations.
  • Responsible for de-escalating participants and guiding persons needing crisis assistance and using training, tools, and sound judgement to minimize challenging or aggressive behaviors.
  • Responsible for monitoring medications as prescribed by medical professionals and following medication monitoring policy.
  • Create and maintain positive relationships with community Hotel and/or Motel providers to include timely responses and billing submission.
  • Responsible for teaching, modeling, practicing, supporting, and articulating the Agency’s treatment philosophy to clients, volunteers, donors and other staff.
  • Responsible for completing and documenting accurate participants intakes.
  • Responsible for communicating agency procedures and expectations, including housing expectations to clients.
  • Responsible for positive and professional interactions with participants, staff, volunteers, donors, and agency partners.
  • Responsible for providing and documenting referrals to other community services, appropriate for client support and care.
  • Responsible for providing education, training, and coaching to participants regarding establishing and maintaining permanent housing.
  • Responsible for learning and accurately inputting data into the Homeless Management Information System (HMIS).
  • Responsible for developing and monitoring, with assigned Participants, an individualized case management plan including identifying any barriers to establishment of permanent housing and methods/activities to address each barrier.
  • Committed to solving problems with participants and other co-workers to ensure a positive and productive working environment.
  • Maintain community relations with key community partnerships including but not limited to law enforcement and community mental health.
  • Support the facilitation and document all safety drills/tests and actual incidents, including critical incidents, emergency plan drills or actual events and shift logs or report.
  • Performs other duties related to the essential duties of the position and within the individual’s capabilities as assigned by one’s supervisor.

  • Associate’s degree and 2 years’ case management experience required
    • Bachelor’s degree in social work or related field preferred
  • 2 years’ experience working with persons with mental issues and/or trauma histories.
  • Lived experience in homelessness strongly preferred.
  • Meet State “Good Moral Character” guidelines.
  • Valid and Current Driver’s License with acceptable driving record.
