Summary
The AC/HH Coordinator will be responsible for facilitating and building ongoing relations with Acute Care (AC) and Home Health (HH) nurses for the purposes of communicating the benefits of Secure Start Services to their patients and facilitating patient consent and enrollment. The coordinator will work in a regional capacity, to support acute care and home health nurses through inbound/outbound communication and field travel. Additionally, the coordinator will work with Sales Specialists, Region Manager, and appropriate KAM roles to develop region specific plans to generate enrollments and increase patient consent. Hollister Incorporated is operating in a hybrid-working environment.
Responsibilities
Secure Start Nurse Participant Relationship Management – 40%
  • Maintain regular contact with Secure Start participating Acute Care and Home Health nurses within assigned territory.
  • Collaborate with Field Sales Specialist and KAM (as required) to ensure consistency of message and to help identify additional enrollment/patient consent opportunity within an account.
  • Product services training and education through telephone support and formally, through on-site presentation as indicated.
  • Provide additional support on issues/concerns as needed.
  • Provide feedback/updates to AC/HH WOC nurses concerning progress/status of enrolled patients.
Non-participant Nurse Education – 20%
  • In partnership with Field Sales Specialist and KAM, identify targeted acute care hospitals/facilities and appropriate acute care nursing contacts.
  • Provide education on Secure Start Services to support AC/HH WOC nurse in discussing the service with her patients.
  • Provide additional follow-up (samples, documentation, training, etc.) as needed.
  • Manage all regional inbound nurse calls and answer queue calls as needed.
  • Coordinate patient enrollment process with nurses as required.
Achieve Business Goals and Objectives – 20%
  • Achieve established metrics and goals on a weekly, monthly and quarterly basis.
  • Comply with all HIPPA policies and procedures and attend training as necessary.
  • Develop and execute annual Region business plan. Review progress at a minimum quarterly.
  • Ability to target accounts from distributed reporting.
Team Collaboration and Relationship Building – 20%
  • Partner with field sales to identify key AC/HH targets and develop tactical plans to achieve goals
  • Attend joint Acute Care/HH Coordinator team meetings as necessary.
  • Collaborate with peers at the AC and HH level in addition to the Customer Service Advisor team to identify best practices to support the achievement of Regional and team goals.
Essential Functions of the Role
  • Ability to travel domestically (10-15% of time)
  • Ability to wear a headset
  • Ability to sit for long periods of time (6 – 8 hours)
  • Ability to work greater than 40 hours if need arises
Work Experience Requirements
  • Number of Overall Years Necessary: 2-5
  • Demonstrated ability to exercise judgement independently
  • Previous experience with customer service
  • Excellent communication skills (verbal and written)
  • Goal oriented, Self-starter
  • Ability to develop and execute business plans
  • Ability to collaborate cross-functionality
Education Requirements
  • Bachelor’s degree
Specialized Skills/Technical Knowledge
  • Computer knowledge including Microsoft Word, Excel, Outlook, Web-based applications and Windows-based navigation
  • SAP/CRM experience and CISCO applications desirable but not required
