Holistic Elevation LLC is Hiring a SUD Case Manager (New Castle) Near Wilmington, DE

Duties:
- Assess and evaluate patients' needs and develop individualized care plans
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Provide support to families dealing with addiction issues
- Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care
- Monitor patient progress and adjust care plans as needed
- Advocate for patients' rights and ensure they receive appropriate services and resources
- Document patient interactions, assessments, and interventions accurately and timely

  • Screening: Conduct a comprehensive screening to determine the severity and nature of the individual's substance use, including any co-occurring mental health disorders or medical conditions.
  • Referral: Refer individual that need treatment and encourage individuals to receive services from a substance use community provider.
  • Identify and Break Down Barriers to Treatment: Barriers may include transportation problems, financial concerns, or scheduling conflicts. Identify barrier(s) faced by each client and provide assistance or referrals to resources that can help overcome these barriers.
  • Follow up on progress in treatment: For beneficiaries that have been placed in treatment with a healthcare provider or substance abuse specialist .
  • Participate in Discharge Planning: work with the healthcare provider to coordinate the discharge of the client after their treatment ends.
  • Facilitate Support: facilitate access to peer support groups, recovery communities, or sober living environments to foster a sense of belonging, accountability, and social support.
  • Participation in Family Team Meetings: participate in meetings with the client’s family members to evaluate the progress made as a result of them attending the treatment sessions.

Requirements:
- Bachelor's or Master's degree in social work or related field

- Certified alcohol and drug counselor

or eligible to be certified in the State of Delaware within one year of employment; or certification by a nationally recognized body in addictions counseling or co-occurring counseling;

or five (5) years of clinical experience working in the field of substance abuse treatment.
- Experience working in healthcare setting preferred
- Knowledge of substance abuse/addiction, behavioral health, mental health counseling,
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Excellent problem-solving and decision-making abilities

Job Type: Full-time

Pay: $20.00 - $21.00 per hour

Expected hours: 40 per week

Schedule:

  • Monday to Friday
  • Weekends as needed

Work Location: In person
