Hillcrest Country Estates, a healthcare provider focused on aging adults, has an opportunity for a Clinical Care Coordinator candidate interested in enhancing the lives of aging adults. Designed for a home-like experience with private rooms and family-style dining, we are a national leading model for "culture change" in long-term care facilities. Each of our eight cottages is home to 13 residents who benefit from 24-hour nursing care services. We're structured with universal workers (Elder Assistants C.N.A/CMA) providing the daily care for our residents and a team of nurses overseeing the cares and the physician's orders. The Clinical Care Coordinator supervises and evaluates team members, is part of the multidisciplinary team, and works with the patients and families to provide assistance from admission to discharge. This position will lead a professional team of career minded nurses to assure the highest quality of care is provided. The CCC will collaborate with members of the multidisciplinary healthcare team to plan, manage, or assess patient's treatments. Requirements for Clinical Care Coordinator: Demonstrates the ability to critically think, plan and organize. Serves as a coach and mentor to team members.
