Job Summary:

We are seeking a passionate Social Service Director to lead and oversee our social service programs. As a vital member of our team, you'll assess the social needs of each resident, formulate comprehensive care plans, and connect residents and families to vital community resources. Your expertise will foster a supportive environment and ensure residents receive the emotional and practical assistance they need.

Qualifications:

  • A bachelor's degree in social work, Human Resource Field, or related disciplines like Sociology, Special Education, Rehabilitative Counseling, or Psychology.

  • License/Certification in accordance with state laws and regulations.

  • One year of supervised social work experience in a healthcare setting.

Physical and Communication Requirements:

Your role will encompass a variety of physical tasks and communication needs:

  • Interaction with residents and families to maintain dignity, provide counseling, and facilitate emotional support.

  • Collaborating with interdisciplinary teams to understand and meet residents' unique needs.

  • Building relationships between residents and staff while promoting individuality and choice.

  • Assisting residents in the decision-making process about their healthcare.

  • Documenting interaction and social care components in assessment and care plans.

  • Identifying community services and facilitating their utilization.

  • Compiling social histories to enrich understanding of residents' backgrounds.

  • Observing and reporting changes in behavior, attitude, or personality for timely interventions.
