Job Title: Director of Social Work

Overview: As the Director of Social Work for our nursing home facility, you will play a pivotal role in ensuring the well-being and social welfare of our residents. You will lead the social work department and collaborate closely with other healthcare professionals to address the social and emotional needs of our residents, fostering a supportive and enriching environment.

Responsibilities:

  1. Leadership and Supervision:

    • Provide strong leadership to the department offering guidance, support, and professional development opportunities.
    • Oversee the day-to-day operations of the social work department, ensuring efficiency and effectiveness in service delivery.
    • Conduct regular performance evaluations and provide feedback to team members to maintain high standards of care.
    • Have a regular assignment of social work duties and tasks
  2. Assessment and Care Planning:

    • Conduct comprehensive assessments of residents' social, emotional, and psychological needs upon admission and regularly thereafter.
    • Develop individualized care plans in collaboration with residents, their families, and interdisciplinary team members to address identified needs and goals.
    • Monitor and update care plans as necessary to ensure residents receive appropriate support and services.
    • Participate in care plan meetings with residents, their families, and interdisciplinary team members.
  3. Crisis Intervention and Support:

    • Provide crisis intervention and support to residents and their families during times of emotional distress or significant life changes.
    • Coordinate with external agencies and resources to access additional support services for residents, as needed.
    • Serve as a liaison between residents, families, and the nursing home administration to address concerns and facilitate effective communication.
  4. Advocacy and Education:

    • Advocate for residents' rights and ensure that their voices are heard in matters affecting their care and quality of life.
    • Educate residents, families, and staff members about available social services, community resources, and relevant policies and procedures.
    • Stay informed about current trends and best practices in social work and long-term care to enhance the quality of services provided.
  5. Collaboration and Interdisciplinary Care:

    • Work collaboratively with nurses, therapists, physicians, and other healthcare professionals to develop holistic care plans that address residents' physical, social, and emotional needs.
    • Participate in interdisciplinary team meetings to discuss resident progress, identify challenges, and coordinate interventions to optimize outcomes.
    • Perform regular resident assessments as required by regulation, policy, and as assigned.
    • Assure safe facility admission and discharge of residents.
    • Foster a culture of teamwork and open communication to ensure seamless coordination of care across disciplines.

Qualifications:

  • Degree in Social Work or a related field from an accredited program.
  • Minimum of 3 years of experience in social work, preferably in a healthcare or long-term care setting.
  • Strong leadership skills with the ability to motivate and inspire a team.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Sound judgment and ability to make decisions in complex and sensitive situations.
  • Knowledge of relevant laws, regulations, and ethical standards governing social work practice.
  • Commitment to upholding the dignity, autonomy, and well-being of older adults.
