JOB SUMMARY

The SVP, Home & Community Care (Post-Acute) will report directly to the Enterprise Chief Medical & Clinical Transformation Officer. The incumbent will work collaboratively within a heavily matrixed environment to lead and expand Highmark Health’s post-acute strategy capable of delivering on its long-term vision. This position will be responsible for in building, growing, and maintaining companies in the Post-Acute space.

This individual will be held accountable to deliver a clinical strategy, programs and operating model that effectively engages members in their care and achieves high-quality and cost-effective outcomes. The incumbent will work with Segment leadership to align clinical strategy with current needs and planned growth. The incumbent will partner with enterprise clinical leaders to shape strategy, leverage enterprise clinical programs based on member needs, share program elements and collaborate with strategic providers and vendors.

ESSENTIAL RESPONSIBILITIES

  • Develop and lead the strategic growth plan that builds upon our current in-home capabilities and is consistent with Highmark Health’s Living Health strategy.
  • Lead and accelerate the shift from acute to in-home care, leveraging Highmark’s digital and telehealth capabilities with a focus on improved health outcomes, patient experience, and total cost of care.
  • Identify & integrate in-home programs into our population health strategy.
  • Collaborate and align with cross functional teams within and outside the enterprise clinical organization to optimize operational processes, workflows and technologies that support the growth and effectiveness of in-home care programs and capabilities.
  • Collaborate and work in tandem with Highmark’s strategic provider partners to enhance our ability to keep care in the home.
  • Identify growth opportunities and potential acquisitions/partnerships to expand our in-home care portfolio and strengthen Highmark Health’s position as a leader in this space including existing Joint Venture partners, and future potential service lines.
  • Responsible for collaborating with our post-acute network management team and continue to direct its strategic direction.
  • This person will be responsible for effectively managing and growing the current Healthcare@Home Profit and Loss capability.
  • Continue to scale and optimize the Enhanced Complex Care Management (ECCM) program while integrating the relevant components into the population health strategy.
  • Develop and deploy scalable, cost-effective strategies and operating model to mature and maintain clinical capabilities in alignment with growth inside and outside the Highmark footprint.
  • Represent the clinical point of view on relevant decisions related to the identification, build-out, and management of technology needed to support operational changes in line with overall objectives.
  • Work in concert with Product and Health Experience team, Enterprise Clinical Organization leaders and physician leaders across the markets to monitor ongoing effectiveness of virtual solution delivery where applicable.
  • Ensure collaboration and sense of urgency around clinical improvement and optimization.
  • Perform management responsibilities to include but are not limited to: involved in hiring and termination decisions, coaching and development, rewards and recognition, performance management and staff productivity.
  • Provide other clinical duties as assigned within the segment and enterprise.

EDUCATION

Required

  • Bachelor's Degree

Preferred

  • MHA, MBA or MPH from an accredited university
  • Masters in Business Administration/Management
  • Clinical Degree (nurse, PA, MD, DO)

LICENSES OR CERTIFICATIONS

  • Registered Nurse, Certified Registered Nurse Practitioner or Medical Doctor preferred

EXPERIENCE
Minimum

  • 15 years of progressive experience in Healthcare Leadership with at least 5 years in a senior executive leadership position (the current/most recent position must be one responsible for clinical programs/transformation in a complex, integrated operating environment).
  • Proven track record in developing, implementing, and growing innovative in-home care programs targeted at the rising risk, high risk, palliative and end of life populations.
  • Demonstrated record of clinical and executive leadership.

Preferred

  • 5-7 years Clinical Practice
  • 3-5 years Network Planning.
  • 3-5 years Healthcare Industry
