Job Title: Certified Diabetes Educator for Maternal Fetal Medicine Practice

Location: Phoenix and Peoria, AZ

Job Description:

We are eagerly seeking a Certified Diabetes Educator (CDE) to join our Maternal Fetal Medicine practice with offices in Phoenix and Peoria, AZ. This role is ideal for someone with expertise in diabetes education, particularly in managing gestational diabetes, type 1, and type 2 diabetes during pregnancy. The candidate will play a crucial role in providing specialized education and support to our pregnant patients, enhancing both maternal and fetal health outcomes.

Key Responsibilities:

1. Diabetes Education and Management: Provide specialized education and support to pregnant patients with gestational diabetes, type 1, and type 2 diabetes, including nutritional counseling, glucose monitoring, and lifestyle modification guidance.

2. Collaboration with Healthcare Team: Collaborate closely with physicians, nurse practitioners, and other healthcare professionals to create and implement effective diabetes care plans.

3. Patient Monitoring and Follow-Up: Monitor patients’ progress and adjust education and management plans as necessary, offering continuous support and guidance.

4. Resource Development: Develop and maintain educational materials and resources for patients.

5. Compliance and Best Practices: Keep abreast of the latest diabetes care and education standards, ensuring adherence to national guidelines.

6. Location Flexibility: The role requires sharing time between our two locations in Phoenix and Peoria, AZ, depending on practice and patient needs.

Qualifications:

  • Certification as a Diabetes Educator (CDE).
  • Bachelor’s degree in nursing, dietetics, or a related healthcare field.
  • Proven experience in diabetes education, especially in maternal health.
  • Excellent communication and interpersonal skills.
  • Ability to work both independently and as part of a multidisciplinary team.
  • Familiarity with current diabetes technology (e.g., insulin pumps, CGM systems) is advantageous.
