Responsibilities:

• Administers medications as stated on physicianorders, ensuring strict adherence to Community medication managementdocumentation and procedure, and demonstrating steadfast dedication to residentsafety
• Upon thorough assessment, creates personalizedservice plans tailored to individual care needs while championing residentindependence and dignity
• Cultivates communication between physicians,healthcare providers, and family, fostering effective communication pertaining to the physical, mental, and social well-being of ourresidents
• Contributes toward quality assurance andperformance improvement initiatives
• Engages in continuous learning and developmentopportunities through attendance and participation in educational seminars,staff meetings, and other learning experience

Qualifications:

• Current state licensure as Licensed PracticalNurse (LPN)
• Current CPR certification
• Exceptional communication and interpersonalskills
• Must demonstrate the ability to serve with love,compassion, and dignity
