JOB SUMMARY
This is a part time position.
The Registered Homecare Nurse provides professional nursing care to assigned clients in a home care environment. This position is primarily responsible for assessing client care needs and implementing appropriate action in a timely manner.

MAJOR RESPONSIBILITIES
1. Understands and adheres to Heritage Community of Kalamazoo policies and procedures.
2. Assess patients’ progress and record observations in the patient’s record each visit
3. Perform evaluation tasks, including vital signs, pain assessment and medication review
4. Administer medication as prescribed by the patient’s Physician
5. Performs examinations and prescribed procedures
6. Operates and monitors medical equipment as necessary
7. Dress or redress open wounds and assess progress of healing
8. Listen to the patient and respond to concerns or requests
9. Coordinate with OT, PT, Physicians and anyone else involved in the patient’s care plan.
10. Promote clients’ independence by teaching clients and their supports to understand conditions, medications, and self-care skills
11. Complete and turn in all paperwork timely and accurately
12. Attend and complete all mandatory trainings
13. Utilize professional judgement to make immediate changes to client care plans and notify supervisor of changes
14. Maintain confidentiality of client information at all times
15. Responsible for assuring client safety
16. Observe universal infection control procedures
17. Responsible for documentation of all client visits
18. Communicate client care changes effectively, appropriately, and timely
19. Maintains a professional appearance and behavior
20. Performs other duties as assigned by the Client Care Manager
REQUIRED CERTIFICATIONS, LICENSES, AND REGISTRATIONS
RN in the state of Michigan.
Must have a minimum of one-year nursing experience.
Physically able to perform duties of a registered nurse. Demonstrates a high commitment to customer service.
Possession of a Michigan driver’s license. Liability insurance is recommended.
Current CPR.
KNOWLEDGE AND CRITICAL SKILLS / EXPERTISE
Must have excellent communication and time-management skills. Can organize, maintain, and fill out records and reports completely and accurately. Has the ability to use critical thinking skills in stressful situations. Able to stay organized and plan out days, events and duties. Capable of using good judgement in assessing problem situations and when to involve supervisory staff, emergency services and law enforcement agencies when appropriate. Compassionate and friendly demeanor. The ability to travel to multiple patients per day.
ESSENTIAL PHYSICAL FUNCTIONS
Work is performed primarily in individual homes. Must be willing to be flexible with time in order to accommodate client care needs. Must be able to safely lift, transfer and ambulate clients. Ability to do moderate bending, lifting and standing on a regular basis, and occasionally lift up to 50 lbs.
