MajorResponsibilities

The Licensed Nurse will:

• Providescare to residents as planned by the interdisciplinary Care Plan Team and usessafe techniques which are in accordance with the Upjohn Community Care Centerand regulatory policies and procedures.
• Assiststhe resident to achieve and maintain their highest level of functioning inaccordance with the care plan and provide input to the Nursing Supervisor forappropriate care plan changes.
• Consistentlyworks cooperatively with the Interdisciplinary Care Plan Team and families todevelop, implement, and review plans of care. Maintains the resident recordutilizing the electronic medical record.
• Must useacceptable infection control procedures including Universal Precautions whenproviding care to residents.
• Cares formedical equipment and supplies used for residents to ensure proper care and chargesare made
• Accuratelycompletes documentation in the medical record to reflect care rendered andreports significant changes in resident condition to the provider uponcompletion of the change of condition assessment or focused assessment usingthe call guidelines provided by Borgess Extendicare.
• Attendstraining programs and in-services to meet Heritage Community of Kalamazoo, the Upjohnand regulatory guidelines, including but not limited to annual skills fair andskills check.
• Maintainsa positive attitude and contributes to the facility interdepartmental team bycompleting assignments in a timely, accurate and professional manner.
• Complieswith HCK Customer Service Standards and the Upjohn’s- CNA Standards of Care.
• FollowResident Rights at all times.
• Assistnew employees in following established Heritage Community of Kalamazoo andregulatory policies and procedures.
• Reportsinoperable, faulty, or unsafe conditions, equipment, and furniture to NursingSupervisor.
• Must beable, during a facility emergency/disaster, to report to an assigned area,which will be assigned by their supervisor, and perform the necessary tasks toensure the calm, orderly and safe handling of residents, staff and public toeither move them to safety and /or remove from the threat causing theemergency/disaster.
• Otherduties as needed or assigned by supervisor.

RequiredCertifications, Licenses, and Registrations

• Must be aLicensed Practical Nurse (LPN) in the state of Michigan
• Unencumberedby any licensing or state violations
• Must alsomaintain the required CEU’s

• Must be able to read, write andcomprehend English.
• Must be able to follow written andoral instructions.
• Must be able to safely lift, transferand ambulate residents.
• Must possess psychosocial skills tobe able to interact effectively with residents, staff, families and visitors.
