Herald Christian Health Center is Hiring a Patient Navigator -Social Services Bilingual, English/Spanish OR English/Chinese Near El Monte, CA

I. Job Summary

A Patient Navigator (Social Services) is a person with direct knowledge of the local community who functions as a “guide” and offers assistance to patients in navigating the healthcare system and accessing social services to improve health outcomes. The Navigator performs a variety of health education, clerical and medical support services within the clinic setting and in the community. The Navigator will assist patients and family members by identifying their needs and linking them to resources.

*This position is grant-funded and contracted through November 30, 2025.

II. Essential Functions

1. Reduce barriers that keep patients from getting treatment and social services support by identifying patient needs and connecting them to resources (i.e. financial assistance, food access, transportation, etc.).

2. Meet with patients to stress the importance of consistent treatment and follow-up.

3. Provide health education and promotion to patients.

4. Coordinate with other HCHC departments for patient referrals and care.

5. Document services provided for each patient and track completed referrals and referrals that are still outstanding.

6. Stay current on available patient resources.

7. Participate in staff meetings and community outreach events.

8. Provide patient updates to Program Coordinator and Project Manager.

9. Provide translation assistance for patients when needed.

10. Perform additional duties as assigned.

III. Organization Expectation

3. Maintain a positive, respectful and professional attitude with all work-related contacts

4. Communicate regularly with the C-Suite about work concerns

5. Meet productivity standards and performs duties as workload necessitates

IV. Education/License/Certification:

1. High school diploma or GED required. Bachelor’s degree preferred.

V. Skills and Specifications:

1. Previous experience in a healthcare or social services setting preferred.

2. Knowledge of community resources available in Los Angeles County.

3. Ability to work collaboratively with staff and patients from diverse socio-economic, ethnic, and cultural backgrounds.

4. Communicate effectively orally and in writing.

5. Committed to ongoing learning that supports individual development and community capacity-building.

6. Demonstrate maturity, good interpersonal skills, integrity, and professionalism when working with patients and team members.

7. Maintain strict adherence to HIPAA guidelines and confidentiality with patient information.

8. Ability to speak, write, and read English fluently.

9. Fluency in a second language (Spanish, Mandarin, or Cantonese) is required.

10. Ability and willingness to work some evening and weekend hours.

11. Ability and willingness to travel to different work sites.

12. Proficiency with Google Suite and Microsoft Office.

VI. Physical Requirement

1. Physical effort which may include occasional light lifting to a 25-pound limit, and some

bending, stooping or squatting. Considerable walking may be involved. The ability to sit or

stand for extended periods of time is required.

employee to successfully perform the essential functions of this job. Reasonable

3. While performing the duties of this job, the employee may be required to work at all HCHC

facilities as needed.

Job Type: Full-time

Pay: $21.00 - $25.00 per hour
