Family Stabilization Case Manager


Hours: Monday - Thursday

Status: Full Time

Reports to: FHS Supervisor/ Manager


Purpose:

Under the direct supervision of the FHS Supervisor, this position provides direct case management for Anthem members for services in homeless prevention, applications for rental and/or utility assistance, Rapid re-housing due to financial hardship.


Duties, Functions and Responsibilities:

  • Conduct initial interview/intake and client needs assessments based on referrals received.
  • Determine eligibility for homeless prevention, rent and utility assistance and RRH based on criteria set in grant.
  • Collect appropriate documents from client(s) to properly document eligibility (Identification of all household members, income, crisis/need, lease, sustainability, MCO designation, etc.)
  • Must be aware of community resources so that correct referrals and information is given to clients to best meet their individual needs.
  • Provide case management and supportive services to clients participating in housing program.
  • Create individualized case plans with each adult member of the household, based on clients' needs, with the goal of achieving self-sufficiency.
  • Review and update client's case plan monthly, and track outcomes.
  • Connect client with wrap-around services through other programs offered at HELP, including WORC, Diaper Bank, holiday programs, etc.
  • Connect client with mainstream programs, including but not limited to, Medicare, WIC, TANF, SNAP SSI/SSDI.
  • Provide client with referrals and resources for other social services and partnering agencies.
  • Enter case notes in true time data (daily) into HMIS, reflecting activities of the client's progress.
  • Maintain current and accurate client files.
  • Maintain clear boundaries with all clients and providers.
  • Must never allow yours or the organization's ethical standards to be compromised.
  • Other duties, as may be assigned.


Knowledge, Skills and Abilities:

  • Knowledge of local social service agencies and mainstream programs.
  • Knowledge of social service work, interviewing/referral techniques.
  • Experience and understanding of homeless issues.
  • Understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.
  • Must communicate effectively with persons from a variety of social, cultural and economic backgrounds, and possess a demonstrated understanding and sensitivity to culturally diverse populations.
  • Effective written, verbal and listening skills. Independently initiates and responds to correspondence.
  • Ability to maintain order and exercise appropriate judgment in crisis situations.
  • Must be flexible and have the ability to quickly adjust to shifting priorities.
  • Establish and maintain cooperative working relationships with consumers, fellow employees, other agencies and the general public.
  • Organize work schedule and effectively manage time.
  • Understand and follow all agency policies and procedures.
  • General computer knowledge.
  • Ability to work flexible hours, as required by consumer case load.
  • Ability to perform accurate mathematical equations to calculate client's rent and utility costs, and to request funding to pay those bills.


Qualifications, Education and/or Experience:

  • Bachelor's degree in Social Work or related field, or combination of education and experience, substantially equivalent to a bachelor's degree.
  • Experience working with homeless individuals, families and children.
  • Must pass a background check and pre-employment drug screening.
  • Must have a current NV driver's license and clear DMV record. Driver History Reports are required to be submitted on a quarterly basis.
  • Bilingual preferred.


Working Conditions:

Work is performed in an office setting, and at various community and residential locations. May be subject to sitting, standing, walking, bending, squatting, lifting, pushing and pulling, and simple grasping.
