Help Them Home Intensive Case Manager

Hours: Monday - Thursday

Status: Full Time; Salaried Non- Exempt

Reports to: Adult Housing Supervisor

Purpose:

Under the direct supervision of the Adult Housing Supervisor, this position provides direct case management services to chronically homeless adults.

Duties, Functions, and Responsibilities:

• Conduct initial interview/intake, and client needs assessments.
• Provide support and arrange services to clients moving into long term housing, to promote transition from homelessness to residential stability.
• Effectively explain public social services programs and assist clients with the application and follow up, to assure services are received.
• Must be able to communicate effectively with persons from a variety of social, cultural, and economic backgrounds.
• Create client centered treatment plans that reflect the needs and desires of the client.
• Complete monthly Treatment Plan Reviews with the client, and obtain signatures.
• Monitor client's progress and provide follow-up services.
• Enter case notes in true time data (daily) into HMIS, reflecting activities of the client's progress.
• Maintain audit ready files at all times. (Random audit checks are regularly performed)
• Maintain clear boundaries with all clients and providers.
• Provide transportation to clients to ensure that all appointments are kept.
• Attend meetings and trainings, as required.
• Other duties, as may be assigned.

Knowledge, Skills, and Abilities:

• Knowledge of local social service agencies and programs, as they pertain to the local homeless population.
• Knowledge of applicable statutes, ordinances, and codes.
• Knowledge of social service work, and interviewing/referral techniques.
• Experience and understanding of homeless issues.
• Understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.
• Communicate effectively with persons from a variety of social, cultural, and economic backgrounds, and possess a demonstrated understanding of and sensitivity to culturally diverse populations.
• Effective written, verbal, and listening skills. Independently initiates and responds to correspondence.
• Ability to maintain order and exercise appropriate judgment in crisis situations.
• Is flexible and can quickly adjust to shifting priorities.
• Establish and maintain cooperative working relationships with clients, fellow employees, other agencies, and the general public.
• Organize work schedule, and budget time.
• Understand and follow all agency policies and procedures.
• General computer knowledge, and the ability to use Microsoft Office software.
• Ability to work flexible hours, as required by client case load.
• Ability to perform accurate mathematical equations, to calculate client's rent and utility costs, and request funding to pay those bills.

Qualifications, Education, and/or Equivalent Experience:

• Bachelor's Degree in Social Work or High School Diploma/GED with (2) two years of experience, substantially equivalent to a Bachelor's Degree, or combination of education and experience.
• Experience working with the homeless population.
• Must be able to pass a background check and pre-employment drug screening. Random drug screening is performed.
• Must have a current NV driver's license and clean DMV record. Driver History Reports are required to be submitted on a quarterly basis.
