Shannon West Homeless Youth Center (SWHYC)

Data Entry Intake Specialist

Hours: Varies

Status: Full Time - Salaried Non-Exempt

Reports to: Shannon West Homeless Youth Center (SWHYC) Director


Purpose:

Under the direct supervision of the SWHYC Director, this position will be responsible for interviewing and assessing applicants, ensure data quality standards, and to input information into the various data bases to include Homeless Management Information System (HMIS).


Duties, Functions, and Responsibilities:

  • Conducts face to face assessment to screen potential participants for program enrollment.
  • Determines if a potential participant is an appropriate fit for the program.
  • Completes intake paperwork with detailed accuracy.
  • Enrolls participants into the appropriate SWHYC program according to grant requirements.
  • Maintains all computer records in the data system, including, but not limited to, the inputs of client demographics and services provided.
  • Creates detailed intake notes, and documents into the data system.
  • Uploads documents into the data system per program standards.
  • Creates new intake files, and submits the file to the case manager supervisor for review.
  • Corrects data deficiencies.
  • Completes status assessment when required.
  • Provides a wide variety of referral information to clients.
  • Maintains SWHYC resident files, controls and monitors the movement and accuracy of all resident files.
  • Monitors and tracks required information for monthly reports and summaries.
  • Maintain complete and accurate information of participants records.
  • Provides crisis intervention as needed.
  • Maintains record room, this includes archiving files when appropriate.
  • Report all program violations within 24 hours of incident.
  • Responds to emails in an appropriate and timely manner
  • Responsible for the storage of program generated paperwork.
  • Creates and maintains medical provider data and follow's up appropriately.
  • Supervises residents' ingestion of prescribed and over the counter medications, and documents medication compliance.
  • Completes all required paperwork and logs, including incident reports, runaway reports, and resident tracking log.
  • Provides transportation to appointments when necessary.
  • Attends scheduled trainings.
  • Assist with hourly rounds when necessary.
  • Assist in the facilitation of daily life skills groups, when necessary.
  • Assist in the daily preparation of meals, when necessary.
  • Assist with resident follow-up activities, as needed.
  • Other duties, as may be assigned.


Knowledge, Skills and Abilities:

  • Knowledge of local social service agencies and programs, as they pertain to homeless and at risk youth.
  • Knowledge of social service work, and interviewing/referral techniques.
  • Experience and understanding of homeless, at risk youth issues.
  • Communicate effectively with persons from a variety of social, cultural, and economic backgrounds, and possess a demonstrated understanding and sensitivity to culturally diverse populations.
  • Effective written, verbal, and listening skills. Independently initiates and responds to various correspondence.
  • Ability to maintain order, and exercise appropriate judgment in crisis situations.
  • Is flexible and can quickly adjust to shifting priorities.
  • Establish and maintain cooperative working relationships with residents, fellow employees, other agencies, and the general public.
  • Organize work schedule, and budget time.
  • Understand and follow all agency policies and procedures.
  • Experience with data entry and HMIS.
  • General computer knowledge, and the ability to use Microsoft Office software.
  • Ability to work flexible hours, as required by the program.


Qualifications, Education and/or Equivalent Experience:

  • High School diploma and 2 years of experience working with homeless or at risk youth.
  • Data Entry and HMIS experience required.
  • Must pass background check, Child Abuse and Neglect background check and pre-employment drug screening. Random drug testing is performed.
  • Must have a valid NV driver's license and clean DMV record. Driver History Reports are required to be submitted on a quarterly basis.
  • Must have a current Clark County health card for food handlers.
  • Must have current CPR/First Aid certification.
  • Must pass annual TB test.
  • Must submit a fitness for duty statement from a physician, prior to employment start date.
  • This position is required to be on site.


Working Conditions:

Work is performed in an indoor/outdoor environment, with driving required at times. May be subject to sitting, standing, walking, bending, squatting, lifting, climbing, pushing, pulling, and simple grasping.
