Description

HeartShare St. Vincent’s Services (HSVS) empowers youth and families to overcome the challenges of homelessness, family crises, addiction, mental illness and poverty. Founded in 1869 as St. Vincent’s Services in Downtown Brooklyn, we began as a home for “poor and friendless boys.” In 2014, HSVS was formed through an affiliation with Heart Share Human Services of New York. Through the affiliation, HSVS developed into a multi-program nonprofit agency with four divisions: Foster Care & Preventive Programs, Integrated Health Services, Housing & Residential Services and Youth & Community Programs. Across our agency, staff members serve our clients with compassion, respect and in collaboration with our outside partners. Our commitment to supporting vulnerable young people and their families in their quest to thrive and reach their fullest potential, ties together our rich array of services. HSVS understands that to best support our clients, our staff must have also be provided with the resources they need to do their jobs, while ensuring self-care. HeartShare is looking for a Socio Therapist to join our Foster Home Program

Responsibilities:

  • Creates, in partnership with families, plans which address safety, behavioral, psychological and permanency needs.
  • Monitors safety, well-being, and permanency progress by conducting regular casework contacts and providing the necessary services to meet the needs of the children and families.
  • Complete timely documentation of all reports and casework activities including FASPS, Psychosocials, Adoption Home studies, Kinship Home studies, court reports
  • Actively participates in Family Court Hearings, Family Team Conferences, and other meetings to advance permanency for children and families.
  • Provide crisis intervention as needed
  • Implement evidence-based models with foster parents, parents, discharge resources and youth.
  • Coordinate care with other agency and community-based programs such as Behavioral Health & Wellness and Health Homes
  • Provide family planning counseling to youth and document discussion with foster parents
  • Work to identify family resources and support for youth through Family Finding evidence-based practice.
  • While performing the responsibilities of the job, the employee is required to frequently communicate with clients who have inquiries about services that you are providing. Must be able to exchange accurate information in these situations

Qualifications


Qualifications:

The Socio Therapist must have:

  • Bachelor's Degree required preferably BSW
  • Experience supporting families and adolescents with social services. Must have a commitment to work from a strength-based, youth development and trauma informed perspective.
  • Ability to work with diverse population of clients and team members
  • Must be highly organized with ability to meet program deliverables and deadlines
  • Must have excellent verbal and written communication skills
  • Ability to work flexible hours - some evenings as needed to meet client needs
  • Ability to use public transportation to travel throughout New York City or Agency vehicle when available
