POSITION Resident Care Manager

DESCRIPTION

Position Summary : The Resident Care Manager will work directly with the Health Services Director and is a key member of an interdisciplinary team that will ensure quality care and delivery of services to the residents living in the community.

Monday-Friday

Salary Range $60,000.00- $65,000.00 DOE

Essential Duties and Responsibilities :

The following is an outline of responsibilities and not intended to be a comprehensive list)

• Develop resident Service Plans and participate in care conferences.
• Input new resident data into Point Click Care and update current resident records.
• Coordinate care with outside providers and maintain appropriate documentation.
• Perform resident evaluations in a timely manner e.g., Pain, Diabetic, Self-Medication, etc.
• Respond to resident / family concerns identified and reported by Caregivers and Med Techs.
• Participate in providing continuing education to staff, as assigned.
• Maintain the Acuity Based Staffing Tool.
• Investigate, evaluate, and document incidents or accidents involving residents and ensure follow-up and update service plan accordingly.
• Work collaboratively with care staff and management team.
• Update ADLs and communicate changes with physicians, care staff and families as needed.
• Follow-up on lab reports and doctor orders, as per policy.
• Other duties as assigned.

POSITION REQUIREMENT

Qualifications :

• Previous experience in a community based care setting
• Excellent written and verbal communication skills
• Demonstrated leadership skills
• Minimum two years experience in an RCC or RCM role
• Point Click Care experience required
