Our Mission

Hearth is a non-profit organization dedicated to the elimination of homelessness among the elderly. This mission is accomplished through a unique blend of prevention, placement, and housing programs all designed to help elders find and succeed in homes of their own. To this end, all housing operated by Hearth provides a creative array of supportive services that assist residents to age with dignity, regardless of their special medical, mental health, or social needs. Hearth believes these goals are best accomplished through respect for elders and staff, with the desire to see both achieve their highest degree of potential.

Overview
The Case Manager/Housing Advocate at Hearth's Outreach Department is central to our mission of ending elder homelessness. This role involves identifying and assisting homeless and at-risk elders in accessing permanent housing and essential services. Responsibilities include outreach, case management, benefits assistance, and coordination of support services. With a focus on compassion and efficiency, the Case Manager/Housing Advocate plays a crucial role in helping elders secure stable housing and rebuild their lives.

Major Functions

- Assist homeless and at-risk elders in finding or maintaining permanent housing.

- Provide necessary case management.

Duties and Responsibilities

Outreach and Case Management

- Identify and assess homeless elders in areas of mental health, substance abuse, cognitive skills, and physical health problems, facilitating the transition to appropriate community residences.

- Conduct case management and develop individualized case plans.

- Assist with benefits application process.

- Help clients access necessary services such as bill payment, homecare, healthcare, social services, and mental health services.

- Assist with moving and transitioning into housing.

- Provide weekly outreach in Boston area shelters.

- Offer stabilization services to housed clients.

- Conduct monthly and as-needed home visits.

- Attend case conferences and scheduled meetings.

Record Keeping

- Enter client information into eHana and Clarity databases on a semi-monthly to monthly basis and as needed.

- Maintain written records of services rendered.

- Collect and maintain records of documents necessary to help clients obtain housing and/or services.

- Be familiar with and perform duties in the following systems with the City of Boston: CAS, Warehouse.

Supervision/Meetings

- Participate in and attend weekly team meetings.

- Meet weekly with Assistant Program Manager and Licensed Clinician to discuss cases and other issues as they arise.

Key Qualifications, Skills, and Abilities

- Strong analytical, problem-solving, and organizational skills.

- Initiative and ability to work independently as well as part of a team.

- High integrity and ability to perform well under pressure.

- Excellent communication and interpersonal skills.

- Ability to represent the organization professionally to the community, vendors, and internal personnel.

- Proficiency in database management and record keeping.

- Bachelor's degree/ Associate’s Degree in social work, psychology, or a related field preferred.

- Flexible and adaptable to changing priorities and client needs.

- Typical full-time work schedule is Monday through Friday, with employment contingent upon successful completion of pre-employment background checks and professional references.
