Social WorkerWhat You Must Have:Bachelors or Master’s Degree in Social Work from a State accredited programCurrent State license required in accordance with State regulationsValid driver’s license and car insuranceOne year of experience as a Social Worker in a clinical care setting required, hospice experience preferredFull Time Employee BenefitsCompetitive PayMedical, Dental & Vision insurancePaid Time OffPaid holidays401k with up to 4% employer matching Tuition reimbursementCompany car for qualifying individualsMileage reimbursementWhat You Will Do:Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.Facilitate community resources based on patient/family needs.Working with Heart'n Soul HospiceOur purpose is to ensure that appropriate end of life health care is available to ALL members of the metro-Nashville area, particularly those traditionally underserved in our city.It is this Agency's policy to provide equal employment opportunities without regard to age, race, color, religion, military status, gender preference, sex, marital status, national origin or disability.
