Position Summary: HealthFlex is seeking a compassionate and experienced Registered Nurse (RN) Manager - Home Health to join our team and ensure that patient care is coordinated and managed appropriately. By joining our team, you will get to use your expertise to help ensure that our community will get the quality home health care they need to improve their quality of life and well-being, one person at a time.
- Receives case referrals. Reviews available patient information related to the case, including disciplines required to determine home care needs. Assigns appropriate clinicians to case.
- Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is generally available at all times to assist clinicians as appropriate.
- Reviews patient's clinical diagnosis, medications, procedures and clinical course.
- Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care
- Attends case conference meetings with organization personnel to facilitate coordination of care.
- Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director/Director of Patient Care Services and appropriate organization personnel.
- Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals. Assists in the orientation of new organization personnel.
- Assists Clinical Director/Director of Patient Care Services in the planning, implementation and evaluation of inservices and continuing education programs.
- Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.

Pay Range: $125K to $145K
Compensation is determined by the following factors; experience, knowledge, skills, location, as well as internal equity and alignment with market data.

Requirements
- Current California RN license.
- Minimum of 2 years of Home Health supervisory experience
- Knowledge and experience in case management and coordination of care in home health
- Excellent oral and written communication skills with leadership experience
- Knowledge of Title 22, Medicare Conditions of Participations, and The Joint Commission Accreditation standards for home health.
