Program Coordinator - Health Risk Assessments

HealthCare Support is actively seeking a Program Coordinator - Health Risk Assessments to fill an opening with a Managed Care Company in Orange, CA.

This is an IN OFFICE role which will require you to come on site.

Daily Responsibilities for Program Coordinator - Health Risk Assessments :

• Support the members in completing their health risk assessment (HRA)
• Ensure communication of the member’s HRA and care plan with the member, primary care provider (PCP) and health care team.
• Identify barriers to member’s care and assist in improving these barriers for all levels of care.
• Work closely with the PCP and the health care team to ensure member access to timely services and coordination of care.
• Shift : M-F, any 8 hour shift between 7am-8pm, will most likely work 8 : 00-5 : 00 PST
• Assignment length : 6 months; could extend

Required Qualifications for Program Coordinator - Health Risk Assessments :

• High school diploma or GED
• 1 year of experience working with the needs of seniors or persons with disabilities (SPD) in a customer / member service capacity required.
• 1 year of experience completing Health Risk Assessments
• 1 year of experience working telephonically

Preferred Qualifications for Program Coordinator - Health Risk Assessments :

• Bachelor's Degree in healthcare management or related field
• Experience working for another Managed Care Organization
• Bilingual in English and (Arabic, Farsi, Chinese, Korean, Spanish, Vietnamese)

Pay Details : $23.00 - $31.05 an hour
