Field Long Term Care Manager (RN / LCSW)
HealthCare Support is actively seeking a Field Long Term Care Manager (RN / LCSW) to fill an opening with a Managed Care Organization in Greensboro.
Daily Responsibilities for Field Long Term Care Manager (RN / LCSW):

• This licensed professional will be responsible for the oversight and coordination of long term support services / long term care for special (higher acuity) populations residing in the community and/or skilled nursing facilities. Examples include but are not limited to: individuals with 2 or more, higher risk medical or behavioral health conditions (e.g., COPD and Schizophrenia). This position assumes a leadership role with internal and external partners to achieve optimal health care outcomes for the member through effective coordination of care activities.
• Manages an active caseload based on state mandated ratios according to residential setting, case intensity and acuity.
• M – F 8 – 5 50% field work

Required Qualifications for Field Long Term Care Manager (RN / LCSW):

• RN, LCSW, LCSW-A licensed in NC
• 2 years of case management experience
• Physical health experience is required (not looking for behavioral health candidates)
• case load number and types of patients must be in the resume
• Home Health, Long Term Care or discharge planning exp highly preferred
• Must be comfortable going into members’ homes for assessments
• Must have experience with EMR
• Must have a Valid and Active Driver’s license
• Must have MS Office exp - excel, outlook, word
• Strong tenure - please include reasons for leaving for positions that are less than a year
• Not working another position alongside this role
• Please ensure candidates’ ENTIRE address and highlight all case management experience in the resume

Preferred Qualifications for Field Long Term Care Manager (RN / LCSW):

• Managed Care experience
• Bilingual

Pay Details: 35/hour - 40/hour
