HealthCare Support Staffing is Hiring a Care Coordinator ( Field based ) Near Uvalde, TX
- Conduct screening and risk assessment interventions per program guidelines
- Determine needs and eligibility of members for various services through field-based collaboration
- Shift M-F 8-5pm
- Remote/home field visits in Uvalde, Crystal City, and Carrizo Springs TX
- 2 years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions.
- Bachelor’s degree in Social Work, Health Services, Behavioral Science or related field or if they have 5 years of experience HS diploma or GED ok
- Microsoft Office, excel
- Clear and active driver’s license, reliable transportation, and insurance
- Service Coordination, Case Management or Nursing experience preferred
- Immediate enrollment in Health Insurance
