Hospice Care Social Worker Job Description:

• Provide psychosocial education to patients and family caregivers about coping skills, hospice and palliative care philosophy, and nonpharmacological symptom management strategies
• Facilitate psychoeducational support groups
• Provide in-services to other service providers and organizations
• Lead community education workshops
• Plans for discharge, coordinates care, and helps clients navigate systems; identifying and linking clients with resources
• Facilitates advance care planning and lifespan planning
• Intervenes in crises
• Mediates conflicts within families, clients, and the interdisciplinary team, and between service organizations
• Participates in interdisciplinary team meetings, care planning, and ethics consultations
• Advocates on behalf of the patient and family
• Document social work activities

Requirements:

· 1-2 years Healthcare experience

BSW with one year experience or MSW with no experience.

Compensation

Benefits package include

$60,000 to $70,000 Depending on Experience
