Under the primary supervision of a Registered Nurse and the Program Manager, the LPN dispenses methadone (liquid & tablets) to program participants. LPN also records and accounts for accuracy of ancillary medications dispensed at Opioid Maintenance Therapy (MAT) and is responsible for ordering and receiving medications from authorized agency pharmaceutical vendor(s). The LPN is also responsible for medication accountability and inventory.
Clinic hours: M-F 5:30am-1:30pm & Sat 6:45am-11:30am
LPN Responsibilities:
- Dispense, record, and account for accuracy for ancillary medications dispensed at the MAT facility by entering Doctor’s RX orders in participants dosing history in SAMMS software.
- Perform breathalyzer readings on all participants’ who are observed with obvious alcohol symptoms.
- Conduct TB screening, testing, and reading for H.A.S. patients at the MAT program as well as H.A.S. employees to ensure compliance with TB protocol. Provide referral to outside providers as needed for chest x-rays.
- Record all of the Medical Director’s telephone orders and assure Medical Director’s signature is on all RX orders pertaining to methadone dispensing and code changes, as per agency and program protocol.
- Complete drug experience reports and submit to SUPR weekly Pharmacy Logs – every Monday.
- Maintain nursing notes in participants chart – strictly adhering to administering dosing as PER DR. RX ORDERS.
- Participate in staff groups, meetings and in-service educational sessions – including CEU’s.
- Take and record vital signs at appropriate intervals on all participants who present with problems or when ordered by the Medical Director.
- Provide phlebotomy services and assist in intake process as necessary.
- Facilitate health education awareness groups on various topics to H.A.S. patients on an on-going basis.
- Perform other duties as assigned by Registered Nurse and Program Manager.
- Participate in internal and external audits procedurally and substantively.
- Complete and send DEA 222 forms to order medication. Receive and stock medication delivery.
- Must be willing to work flexible hours as per the clinic’s hours of operations including the possibility of Saturday hours if needed.
- Bilingual (English/Spanish) preferred.
- Possession of a valid Illinois Driver’s License and willingness to use personal vehicle during course of employment.
- H.A.S. requires all staff to work on-site.
- Must have High School diploma or equivalent.
- Must have valid Illinois LPN License.
- Two (2) years’ experience in methadone field strongly preferred.
