LSW School Navigator - Lima City Schools (High School/Elementary)
This is a full-time position during the School Year August through May but paid all year round.
All school-based employees receive 90% of a full-time rate.
(Monday through Friday 7:30am - 4:00pm)
POSITION PURPOSE
This position is responsible for providing case management and outreach services to patients of special populations or with health care needs that are not available within the scope of services of HPWO. Collaborate with parents to obtain consent to ensure needs met.

Bachelor’s Degree in social work, sociology or psychology or related field, or related experience.
REQUIRED KNOWLEDGE:
Successful completion of in-house training.
EXPERIENCE REQUIRED:
Licensed Social Worker (LSW) in State of Ohio required.
Minimum of three years of experience in a mental health or medical field with documented clinical course work and field experience.
SKILLS/ABILITIES:
Demonstrate the ability to identify planning related to patient learning needs and to assess patient’s knowledge, skill level and readiness for learning.
Excellent oral and written communication skills required.
Ability to work with persons form a wide diversity of social, ethnic and economic backgrounds is necessary.
Must be comfortable with presentations in the community.
Ability to demonstrate positive customer service skills.
Must possess ability to cope with complex situations in a professional manner.
Must respect and maintain confidentiality.
ESSENTIAL FUNCTIONS AND BASIC DUTIES:
Assists with the coordination of care for patients as directed by the healthcare providers.
Completes an assessment of patient/family needs with respect to physical, psychosocial, educational and all other needs.
Assesses patients with respect to specialized needs and documents recommendations.
Coordinates and facilitates a medically appropriate plan of care with the patient as directed by the provider.
Identify patients meeting criteria for case management (i.e. high-risk, problem-prone patients) and implement/oversee case management services.
Serves as a resource for the health team, patient, and family.
Advises and makes recommendations to physicians, dentists, patients and families regarding the availability of resources and coverage, locates the resources, and arranges for the provision of cost effective services for patients.
Make appropriate referrals to community resources on behalf of patients and/or family members (ie. patient’s financial concerns, difficult social situations, difficult placement issues, multidisciplinary referrals).
Maintain school referrals including documentation and follow up communication with school staff.
Collaboration with school staff to support student academic and behavioral success within the school environment.
Collaboration with students identified with behaviors resulting in school discipline.
Participate in school meetings regarding identified students as necessary.
Maintains a database of patients served that includes all support services and efforts made to resolve issues of care.
Participate in community outreach by developing working relationships with other agencies whose services will be beneficial to HPWO patients.
Facilitates and coordinates activities with any HPWO assigned support group.
Participates in media events such as health fairs, and other community projects with the goal of increasing patient population and name recognition.
Responsible for performing other duties as required assisting HPWO in achieving its mission.
Participate in quarterly Mental Health and Recovery Services Board meetings.
Participate in continuing education.
Work with potential patient to obtain necessary consent for treatment. Performs other duties as assigned.
