Healing Hearts Homecare is hiring a part time registered nurse to complete field supervisory visits and 60 day assessments . The RN Case Manager consults as needed with the physician and the office giving details about patient care. Performing accurate assessments and ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, updating care plan bi-weekly, completing supervisory visits on CNA/ caregivers, and attending bi-weekly case conference are key responsibilities of this role. We currently have 8-12 visits per week; this position has potential to go to full time after 90 days . Pay is per visit. Mileage is paid from first client to last client. Benefits are available after 90 days should you move to a full time position.

Qualifications:

• Associate's Degree in Nursing (ADN) or above required.
• Clear and active RN Licensure in the state of practice required.
• CPR Certification required.
• Active BLS certification in the state of practice.
• One (1 ) years of experience as an RN in a hospital or geriatric hospital setting required. 2 years of Home Health experience
