At Heal at Home, our registered nurse case managers plan, organize, and direct home care services. They are experienced in nursing and have an emphasis on community health education and independence. Professional nurses build from community resources to plan and direct services that meet the needs of individuals and families.

Responsibilities

• Monitor patient’s condition and assess their needs to provide the best possible care and advice.
• Observe and interpret patient’s symptoms and communicate them to physicians.
• Collaborate with physicians and nurses to devise individualized care plans for patients.
• Perform routine procedures (blood pressure measurements, administering injections, wound care) and document accordingly.
• Adjust and administer patient’s medication and provide treatments according to physician’s orders.
• Foster a supportive and compassionate environment to care for patients and their families.
• Conduct Re-certification, Supervisory and Admission visits.

Requirements:

• Valid Indiana RN License
• 1 year home health experience or 2 years clinical hospital experience.
• Current CPR certificate
• EMR experience
• A team player with excellent communication and interpersonal skills.
• Responsible and compassionate.
• Outstanding organizational and multi-tasking skills.
• Excellent problem-solving skills

Job Type: Full-time

Pay: $40.00 per hour

Expected hours: 40 per week
