Position Overview:
As a Registered Nurse Case Manager, you will be pivotal in coordinating comprehensive care for our home-based clients, utilizing your exceptional assessment skills. This position is perfect for those who are eager to step away from traditional hospital work hours and embrace a rewarding career in community-based nursing, ensuring that clients regain and maintain independence within the comfort of their homes.
Key Responsibilities:
Collaborate with physicians, social workers, and other healthcare professionals. - Provide direct patient care across a diverse client base. - Engage in activities such as wound care, IV therapy, management of feeding tubes, tracheostomies, and catheters.

Requirements:
- Valid Registered Nurse license in Indiana in good standing.
- At least one year of home care experience and case management.
- Current CPR certification
- Proficiency in various clinical procedures (IV, wound care, Feeding Tubes, tracheostomies and Catheters.).
- Excellent interpersonal and communication skills.
- Valid driver’s license and current auto insurance.
